How to make a new Philippine passport

 

Required Documents

  1. Confirmed Appointment
  2. Personal Appearance
  3. Accomplished Application Form
  4. Original and Photocopy of Philippine Statistics Authority (PSA) Authenticated Birth Certificate
    • Married Females (who are using their spouse's last name) must alsi present the Original and Photocopy of the PSA authenticated Report of Marriage
    • Local Civil Registrar copy is required if the PSA Birth Certificate is not clear
  5. Valid ID (Any of the following)
    • Philippine Identification (PhilID) / ePhilID
    • Philippine Identification (PhilID)/ePhilID
    • Social Security System (SSS) Card
    • Government Service Insurance System (GSIS) Card
    • Unified Multi-Purpose Identification (UMID) Card
    • Land Transportation Office (LTO) Driver’s License
    • Professional Regulatory Commission (PRC) ID
    • Overseas Workers Welfare Administration (OWWA) E-Card
    • Commission on Elections (COMELEC) Voter’s ID or Voter’s Certificate issued from COMELEC main office in Intramuros, Manila.
    • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
    • Senior Citizen ID
    • Airman License (issued August 2016 onwards)
    • Philippine Postal ID (issued November 2016 onwards)
    • Seafarer’s Record Book (SRB) or Seafarers Identity Document (SID) (*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)
    • Valid or Latest Passport (For Renewal of Passport)
    • School ID (if applicable)

Steps on how to make a Philippine passport

Step 1: Gather your documents

Step 2: Book an appointment with the DFA for a new passport

Step 3: Bring your required documents to the DFA office during your appointment date

Step 4: Submit your documents to the assigned staff

Step 5: Pay the processing fee

Step 6: Go to take your photo

Step 7: After taking your photo, review the receipt that you have received. It should indicate which date your passport is ready for release

Step 8: Once your passport is released, review if the information printed on the passport is correct

 

How to book an appointment for a new passport

Step 1: Visit the DFA website

Step 2: Click on the Book an Appointment button

Step 3: Click on the Schedule an Appointment button

Step 4: Click on the box to agree to the Terms and Conditions button

Step 5: Select if you want to book an individual appointment or group appointment

Step 6: Select the Region, Country, and Site that is nearest your location

Step 7: Click on the "I confirm that I have read, understood and agree to the above conditions"

Step 8: Click on the "Next" button

Step 9: Select a date and time for the appointment then click on the "Next" button

Step 10: Type in your personal details the click on the "Next" button
